Intake Questionnaire for Prospective Grantees

In partnership with Uber, PayPal and Walgreens, LISC has created the Vaccine Access Fund to support free Uber rides that facilitate vaccination of people in communities hard hit by the pandemic,, particularly people living on low-incomes and Black and Latinx individuals. Organizations wishing to be considered for a grant for ride coordination or the new “Innovation Pool” should please complete the following form. Please consider the following guidance regarding eligibility:

Racial/Health Equity: The Vaccine Access Fund prioritizes grantees that are led by or serve Black, Indigenous and People of Color (BIPOC). Only organizations demonstrating a history of engagement with communities of color and/or low-income communities hard hit by the pandemic will be considered for grants.

Geography: Applicants from any US area are welcome. Availability of Uber drivers, to be confirmed by Uber, and need for transportation resources to support equitable vaccine deployment are additional geographic criteria.

Organization type: Applicants may be 501(c)(3) non-profit organizations, public agencies or for-profit entities with a social purpose.

Timeline: Grants may run through March 2022 or longer as needed.

Organizations who pass an eligibility screening based on the above criteria will be invited to submit additional information as part of a grant application. We will make best efforts to communicate with all groups promptly, but appreciate patience during times of high volume of inquiries.
